Travel Vaccines
When travelling abroad, it is often necessary to have travel vaccinations prior to travelling to protect your health whilst in foreign countries. This page aims to provide you with the information required to be able to adequately supply you with the correct treatment for your travels as well as provide details for the Nurse administering your vaccines. It is advisable to have any vaccinations a minimum of 8 weeks in advance of travel.
Please complete the Travel Risk Assessment Form below and return to the reception at your earliest convenience. Your medical record will be assessed alongside this form, this acts as a prescription from the GPs to enable the nurses to legally administer the recommended vaccines. You will then be contacted to arrange an appointment with the Travel Nurse. If you are travelling within six weeks, we are not able to provide travel advice within your required timeframe. We have been told the below clinics can offer specialist travel advice which may include a shorter timeframe for assessing your travel risk. Please note these may incur additional fees.

Before you can book an appointment we need to know about your travel itinerary and any significant aspects of your health. Therefore you must complete a Travel Risk Assessment form and return it to the surgery for all those who are travelling. The Travel Nurse will then assess the form and decide which vaccinations you require.
Due to the questions that this assessment may raise, you may receive a telephone call from the nurse to discuss the vaccinations she is proposing. If you do not receive a call from the nurse, the reception team will contact you to arrange an appointment.
Most travel vaccinations are not supplied via the NHS and if the nurse has assessed that you require any vaccinations that are chargeable, when you make the appointment you will be asked to pay for your vaccines in full. Payment can be made in the form of either cash or card.
