Register with our practice
Anyone wishing to register should live within our Surgery boundary, eg. Corfe Castle or the surrounding villages.
Registration forms are available from our Reception desk. Alternatively, please print and complete BOTH of the below forms in full. Please note, you will be required to provide photo ID eg. Passport/Driving Licence (over 18 only) and NHS number when registering. You must also return your registration forms in person so that we are able to verify your ID.

Once registered, new patients taking regular medication will be required to book a New Patient Check with one of our GP’s.
Catchment Area (Practice Boundary)
You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.Named GP
You may be aware that from April 2015 all practices are required to provide all patients with a named GP who will have overall responsibility for the care and support that our surgery provides to them.
Dr Lepper will be your named GP. However, this does not prevent you from seeing any other GP in the practice. Any queries regarding this, please contact Kerry Norman, Practice Manager.
