Anyone wishing to register should live within our Practice boundary, eg. Corfe Castle or the surrounding villages. Please see the below map showing our Practice Boundary.
To register with our Practice, please complete the online form which can be found HERE.
Alternatively, if you are unable to complete the online form you may print the below form or collect a copy from our Reception desk.
Data Sharing
Please read the below Data Sharing document before submitting/returning your registration form. If you prefer to opt out sharing of your information, please also return the below Data Sharing Opt Out Form to Reception.
Once registered, new patients taking regular medication will be required to book a New Patient Check with one of our GP’s.

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.Named GP
You may be aware that from April 2015 all practices are required to provide all patients with a named GP who will have overall responsibility for the care and support that our surgery provides to them.
Dr Lepper will be your named GP. However, this does not prevent you from seeing any other GP in the practice. Any queries regarding this, please contact Kerry Norman, Practice Manager.
